HLC Off-Campus Permission Slip

Hope Learning Center allows high school students (grades 9–12) to leave campus during designated break periods between classes to walk to nearby businesses to purchase snacks. To ensure student safety and accountability, all students must check in and out at the HLC office before leaving campus.
We require signed parental/guardian permission to grant this privilege.

Details & Expectations

● Off-campus privileges apply only during the student’s designated break periods
between classes.
● Students may walk to nearby businesses to purchase snacks and must return on time
for their next scheduled class.
● Students must check in and out at the HLC office before leaving and upon returning to
campus.
● Students are expected to behave responsibly and respectfully while off campus.
● Off-campus privileges may be revoked if the student does not follow these guidelines.
● HLC is not responsible for students during the time they are off-campus and outside
direct supervision.

PARENT - I give permission for my child, named above, to leave the Hope Learning Center campus during their designated break periods between classes to walk to nearby businesses, including the gas station, to purchase snacks. I understand that my child is required to check in and out at the HLC office, and that while they are off-campus, they are not under the direct supervision of HLC staff. I further acknowledge that HLC is not liable for any incidents that occur while my child is off-campus. I have reviewed and understand the guidelines outlined above and acknowledge that off-campus privileges may be revoked at any time.

STUDENT - By checking this box you agree to follow all school rules and conduct myself responsibly while off campus. I understand that I must check in and out at the HLC office, return to campus on time, and that this privilege can be revoked at any time for not following guidelines.
